This creates a negative feeling where at lower difficulties, instead of wanting to run the content, you're often better off just selling it to someone who can run it at maximum difficulty.

Instead of using the singular, combined item in the Realmgate, you can now just bring your Splinters. Breach is a type of map encounter. Interacting with a Breach will create an expanding tear in the fabric of reality, causing demons to spill out into the world.

Mechanics

Breach monsters can only be seen and damaged inside of the expanding Breach. The Breach will continue to expand for a specified duration before closing in on itself. Defeating more Breach monsters will keep the Breach open longer.
