Blue Mushroom is a quest item, which can be found in Apex of Filth in Act 3.

It drops randomly upon defeating any monster within the area and is not related to any actual quests, but can be thrown into the cauldron at the Bubbling Respite landmark, if 3 mushrooms have been found.

(The other 2 being Green MushroomGreen MushroomA curious find... and Red MushroomRed MushroomA curious find...)

Usage

If all 3 mushrooms (Green MushroomGreen MushroomA curious find..., Red MushroomRed MushroomA curious find... and Blue MushroomBlue MushroomA curious find...) are in the players inventory, they can be thrown into the cauldron at the Bubbling Respite landmark in Apex of Filth to brew two magic flasks:
