Delirium is a map mechanic. The exile may encounter delirium mirrors. After walking through the mirror, delirium mists will begin spreading throughout the area. Additional delirium monsters will spawn in areas affected by delirium, and monsters will deal more damage and take less damage. The longer the mist lasts, the higher the magnitudes of its effects. Rare and unique monsters will be affected by delirium modifiers that result in mirages manifesting out of them and attacking the exile. Defeating monsters within the mist will build up reward counters, which require more and more monsters to be killed for each stage.

Certain maps may contain a Delirium Mirror near the starting area of the map. Walking through the mirror will create Delirium Fog that spreads and dissipates outwards from that point in a ring shape. While in the fog, Delirium monsters may spawn, and existing monsters deal more damage and take less damage based on the absolute distance from the starting point. The longer and farther the fog travels, the higher the Delirium. Additionally, rare and unique monsters may gain Delirium modifiers, often manifesting as arena hazards or mirages that attack the player or granting benefits to monsters.

Defeating monsters builds up the reward counter that appears near the life globe; after the Delirium encounter ends by leaving the fog for over 5 seconds or manually ending the Delirium, the accumulated rewards will drop in the form of Liquid Emotions (1 random type per reward icon) and Simulacrum Splinter. The quantity of Simulacrum Splinters dropped scales with map tier.

Players are also guaranteed 1 Simulacrum Splinter per reward icon filled.
