Tribal Medicine is a quest in Act 4, begun by picking up the Shark FinShark FinThe sea gave it strength. Now it serves a greater purpose. after defeating the Great White One on Whakapanu Island.

Now it serves a greater purpose. can be taken to Kaimana in Ngakanu (or on the Karui boat in Kingsmarch, if you're already completed Heart of the Tribe) in exchange for a Level 4 Uncut Support GemUncut Support GemCreates a Support GemRight Click to engrave a Support Gem. or level 11 Uncut Spirit GemUncut Spirit GemCreates a Persistent Buff Skill Gem or Level an existing gem to Level #Right Click to engrave a Persistent Buff Skill Gem..

This can be done right away after starting the quest as you can sail to Ngakanu immediately by revisiting Makoru in Kingsmarch.
